Is your home schooler planning to obtain a drivers permit or license? If so, the parent or the student will need to make an appointment with the home school coordinator to obtain a Statement of Enrollment Form.  DMV requires a Statement of Enrollment Form from the home schoolers local ESD and will not provide the student with a permit/license without this form. 

Please note:
If you have not notified Multnomah ESD that you are home schooling your child, you must complete a Notification form.
Unfortunately, your child will not be able to obtain a Statement of Enrollment form until your form has been processed. Your form will be processed in the order in which it has been received (please allow 2 weeks).
